Yes, I agree with the waiver: 20 Performing Arts Academy Release of Liability & Waiver For the purposes of this release of liability and waiver, “I/we” refers to the student and parent/guardian of the student, if the student is under 18 years of age upon registration. I/we realize that participation in music, dance, and theatre classes and activities could involve personal injury/death. Despite precautions being taken by 20 Performing Arts Academy, accidents, injuries or death may occur. By submitting this registration form, I/we assume all risks related to the use of any and all spaces used by 20 Performing Arts Academy. I/we agree to release from responsibility 20 Performing Arts Academy, including all crew, students, teachers, staff members, and facilities used by all entities from any class action, claims, or demands now and in the future. I/we will not hold 20 Performing Arts Academy liable for any personal injury/death or any personal property damage/loss, which may occur on the premises before, during or after classes/rehearsals/performances. Furthermore, I/we agree to obey the company and facility rules and take full responsibility for my/our behaviour in addition to any damage I/we may cause to the facilities utilized by 20 Performing Arts Academy. I understand that 20 Performing Arts Academy is a licensed and insured organization. In the event that I/we should observe any unsafe conduct or conditions before, during or after my/our classes/rehearsal/performances, I/we agree to report the unsafe conduct or conditions to the administrator, instructors or staff members as soon as possible. By submitting registration, I/we indicate that I/we understand and agree to the above statements, and release 20 Performing Arts Academy of any liability.
